Physicians Formula New 2012 Mail-in Rebate Forms Save On Makeup

Physicians Formula has a bunch of new mail-in rebates for 2012. I love their products and am excited to try out some of the new ones. Make sure that you choose the correct ones and that they contain the little pink sticker on them, ensuring there is a rebate available.

To get your rebate:
1) Purchase specified Physicians Formula specially marked product with the mail-in rebate label between 12/01/11 and 11/30/12.
2) Print out, complete, and mail this rebate form along with the Mail-in-Rebate label and your original sales receipt with purchase price circled.
3) Rebate must be postmarked by December 15th, 2012
