To get yours:
1. Fill out the form below and send it together with 4 UPC Codes from participating Villaggio products
2. When your form and UPC codes have been received and verified, you will be contacted by email with confirmation, a unique Staples Calendar Code and a link to the Staples website where you will create your personalized calendar.
3. The Staples website will provide instructions for creating the calendar
4. Your Free calendar will be ready to be picked up at your nearest Staples location.
