To claim your Free Gift Card:
1. Read the Terms and Conditions.
2. Click on the "Print Redemption Form" button below and complete all the fields on the Redemption Form.
3. Mail completed Redemption Form along with the product UPC code and original cash register receipt dated between November 5, and December 31, 2012 with the applicable ASPIRIN® 81mg product (100 tablets or higher) purchase circled to the address indicated on the form.
4. Offer requests must be received by February 1, 2013.
