Danone Activia has a new mail-in rebate offer! Get a Free $10 Gift Card when you collect 5 unique UPC codes from Activia products! Offer valid until February 10, 2014 or while supplies last. Limit 1 per household.

1. Print the form herein and fill it out with all the required information.
2. Stick or staple on this sheet 5 original unique product codes (UPC) from any Activia® packfrom Danone. Only original UPCs will be accepted; all photocopies, reproductions and UPCs of single servings of 100 g will be rejected.
3. Mail your request to:  “Activia Mail-in Offer,  P.O. Box 295, 157 Place D'Armes, Montreal, Quebec,  H2Y 3J2 by February 10, 2014. Requests postmarked after this date will not be accepted. Once your request has been validated, a $10 gift card will be sent to your attention.
